Overview
Gallstone disease is leading cause of abdominal pain causing patients to seek medical care in the United States. Contemporary data suggests that 5-17% of patients who undergo a laparoscopic cholecystectomy (LC) will be found to have common bile duct stones. The management of these patients has traditionally involved the use of endoscopic retrograde cholangiopancreatography (ERCP) either pre- or post-operatively, leading to decreased familiarity with the LCBDE procedure among practicing surgeons. This one-day course will offer participants the opportunity to learn the necessary technical skills to perform a successful LCBDE through didactic sessions and targeted practice on a validated LCBDE simulator that incorporates laparoscopic, endoscopic and fluoroscopic views. The curriculum teaches surgeons how to effectively lead an OR team to perform an LCBDE procedure. Communication with OR technicians and nurses and organization of the team to perform the multi-step procedure safely and effectively are key components of the training. Patients with common bile duct stones are treated with either ERCP or LCBDE. Currently in the United States, >90% of such patients are treated with ERCP, so surgeons have limited exposure to LCBDE during their residency training and independent practice. This in turn leads to limited opportunity to acquire LCBDE-related skills.

Objectives
  1. Have a greater understanding of the clinical impact of choledocolithiasis and the implications on surgical practice. Topics covered will include: Current trends in management; Current surgical training paradigm; Clinical outcomes research comparing LCBDE versus ERCP.
  2. Describe the indications and contraindications for performing a LCBDE in a patient presenting with choledocolithiasis.
  3. Understand and describe the procedural steps necessary to complete a LCBDE: Description of necessary durable equipment: Strategies for training ancillary staff (OR nurses, scrub technicians).
  4. Practice on validated LCBDE simulator. Participants will: Perform a pre- and post-evaluation on LCBDE simulator; Complete post-course written test to assess knowledge of covered topics.
