**Log in with the same credentials each time you access the FSM Office of CME website to prevent duplicate accounts. Duplicate accounts will impact your CME credit transcript, activity registration and evaluation, and disclosure information. If you are logging in with an email and forgot your password, select “Forgot Your Password” rather than creating a new account. Lurie Children’s Hospital (LCH): You may use an NU or LCH email to create an account and sign in with your email and the password you create.

All others: You may use any email and password to create an account.
